Basic Information
Please provide us with some basic information to start the onboarding process.
Physician Referral

Please get in touch with your child’s primary Physician and have them fax a referral.
New Patient Paperwork
Please complete the new patient paperwork. You may complete the paperwork online, and then download it and email it to us.
Insurance Card
Please send a copy of the front and back of your child’s primary insurance card. If your child’s secondary insurance is Medicaid, we also require a copy of the card/stickers.
